Hazel is a Nurse Practitioner with over 15 years of clinical experience, providing care to individuals across the lifespan and in diverse healthcare settings. For the past eight years, her practice has focused on addictions and mental health, supported by advanced training in the comprehensive assessment and treatment of concurrent disorders and substance use disorders, including opioid, stimulant, and alcohol use. Hazel has extensive experience supporting individuals experiencing homelessness, precarious housing, involvement in the sex trade, and histories of justice system involvement. She is deeply committed to a trauma-informed approach that prioritizes safety, trust, and collaboration. Her practice is grounded in evidence-based interventions designed to support recovery, enhance stability, and improve overall health outcomes.
